About Us
A Better Home Health Care Agency
Eden Pathways operates a Comprehensive Home Health Care Agency in the Twin Cities and Metro areas.
The Agency is managed by an actively practicing Physician in the Twin Cities. Currently we serve the following areas: Minneapolis, St. Paul, Plymouth, Wayzata, Minnetonka, Roseville, Orono, Coon Rapids, Brooklyn Park, Brooklyn Center, New Hope, Edina, and all surrounding areas
We provide primarily community-based services that utilize the skills and services of a Registered Nurse (RN), Licensed Practical Nurse (LPN), Personal Care Assistants (PCA), Home Health Aides (HHA), Homemakers, and Companions. We also provide Private Duty Nursing services.
We work together as a team in an effort to restore function, improve mobility, and to enhance our clients overall physical, emotional, mental and cognitive health.

As an actively practicing Physician managing the Agency, we aim to decrease ER visits, hospital admissions, falls, pressure ulcers (bed sores), social isolation, depression and malnutrition; and we institute interventions that increase mobility, muscle strength, endurance, balance, nutrition, and mood.
