Our Services
Skilled Nursing Visits (RN/LPN)
Our RN will be the team member that does the full/initial assessment of all new patients.
They are also tasked with developing the care plan that is specific to the patient needs.
From their assessment they determine what additional services may be needed (for example physical therapy, or the need to apply for medical rides). Our aim is to ensure than an optimal comprehensive care is delivered.
Our LPN’s are the team members that implement the care plan and provide day to day/hands on nursing care to our patients in their homes.
Services may include blood pressure checks, chronic disease management and teaching, medication set ups, daily weights, and dietary teaching. The services may also include INR checks, wound care, infusion therapies, tube feedings, catheter care, venipuncture, ostomy management etc.
PCA's & HHA's
Our PCA’s and HHA's provide care under the supervision of our Registered Nurse (RN).
Their role is to help with day-to-day (activities of daily living); personal care, bathing, meal preparation, cleaning, shopping and safe ambulation.
They are also available to accompany you to medical appointments.
They also provide nutritional, social, and emotional support.
The care provided can be in the patient’s private residence, nursing home (private duty) or during hospitalizations (private duty).
Homemakers/Companions
Homemakers/Companions also perform household services that are essential to help the patient live independently in her/his home.
Services include, meal preparation, light housekeeping, laundry, errands out of home, and grocery shopping/errands.
They are also available at accompany client on social outings/activities
They also provide medication reminders.
Care can be a provided for a few hours a day, overnight hours or up to 24 hours.
