We are currently hiring for the following positions:

POSITION DESCRIPTION –REGISTERED NURSE

 

REPORTS TO:  Director of Nursing

 

PRIMARY RESPONSIBILITY:  The Registered Nurse is responsible for providing clinical services in home health care.    

 

JOB RESPONSIBILITIES:

 

  1. Provides an admission assessment for all new clients:

  • Completes all admission paperwork in a timely manner

  • Prepares a service plan for each client with input from the client and/or responsible party

  • Orients direct care staff to the service plan

  • Obtains physician orders, as appropriate

  • Obtains needed equipment, supplies and medications for clients

  1. Appropriately updates the assessment and physician orders

  2. Delegates activities to the Home Health Aides as appropriate according to nursing judgment and state regulations

  3. Provides supervisory visits according to nursing judgment and state home care licensure regulations

  4. Reviews and updates the service plan to assure that it is current and accurate

  5. Completes discharges and transfers for clients when needed

  6. Establishes and implements the medication management program for clients based on assessed data and according to agency policy and state statute

  7. Documents as indicated in clinical record

  8. Responds to clinical problems as identified by the home health aides

  9. Communicates with families, responsible parties, case managers and other health care providers regarding the clients when indicated

  10. Participates in the orientation of new staff as delegated

  11. Participates with staff training as delegated

  12. Is aware of, uses and promotes good safety practices, including but not limited to:

    1. Body mechanics

    2. Reporting of defective equipment or hazards

    3. Wiping spills or wet floors immediately

    4. Using Universal Precautions and appropriate infection control measures

    5. Knowledge of emergency procedures

    6. Reporting incidents/accidents to the Administrator on the day of occurrence

  13. Participates in care conferences with other members of the health care team

  14. Participates in the agency’s quality assurance program

  1. Demonstrates commitment to team building activities within the agency

  2. Maintains absolute confidentiality of all information pertaining to clients, families, and employees

  3. Promotes a safe and comfortable environment for the client and family

  4. Demonstrates excellent written and verbal communication skills

  5. Recognizes and manages own stresses, which may affect work performance

  6. Participates in in-service educational programs, as required by state regulations

  7. Performs additional duties as assigned by the management team

  8. Adheres to all agency policies

 

 

EXPERIENCE/EDUCATION/JOB SKILLS REQUIREMENTS

 

  • High School Graduate

  • Graduate of accredited nursing program and current license as a registered nurse in the State of Minnesota

  • Experience in home care or assisted living preferred

  • Supervisory experience preferred

  • Must be able to read, write, comprehend and carry out directions and instructions

  • Able to function independently

  • Good health status and emotional stability

  • Demonstrate a strong commitment to service excellence

 

WORKING CONDITION REQUIREMENTS

 

PHYSICAL DEMANDS:  (Occasionally means 1-2 hours of an 8 hour workday, frequently means 2.5 to 5.5 hours of an 8 hour workday and continually means 6 to 8 hours of an 8 hour workday)
