We are currently hiring for the following positions:

POSITION DESCRIPTION – DIRECTOR OF NURSING/NURSE MANAGER

 

 

REPORTS TO:  Administrator

 

PRIMARY RESPONSIBILITY:  The Director of Nursing is responsible for the clinical services in home health care.    

 

JOB RESPONSIBILITIES:

 

  1. Provides or delegates an admission assessment for all new clients:

  • Completes all admission paperwork in a timely manner

  • Prepares a service plan for each client with input from the client and/or responsible party

  • Orients direct care staff to the service plan

  • Obtains physician orders, as appropriate

  • Obtains needed equipment, supplies and medications for clients

   2. Assures that the assessment and physician orders are appropriately updated

   3. Delegates activities to the unlicensed personnel as appropriate according to nursing judgment and state regulations

   4. Provides or delegates supervisory visits according to nursing judgment and state home care licensure regulations

   5. Reviews and updates the service plan to assure that it is current and accurate

   6. Assures the completion of discharges and transfers for clients when needed

   7. Establishes and assures the implementation of the medication management program for clients based on assessed      data and according to agency policy and state statute

   8. Documents as indicated in clinical record

   9. Responds to clinical problems as identified by the unlicensed personnel

   10. Communicates with families, responsible parties, case managers and other health care providers regarding the           clients when indicated

   11. Directs in the orientation of new staff

  • Completes competency evaluation

  • Provides orientation to clinical responsibilities

   12. Conducts the annual performance evaluation of direct care staff

   13. Responds to employee performance problems or complaints related to clinical issues

   14. Participates with the Administrator in disciplinary activities as needed

​

   15. Oversees the staff education program:

  • Provides in-services as appropriate

  • Coordinates other educational opportunities for staff

  • Obtains relevant training materials

  • Provides documentation of training to Administrator

  • Responds to requests for training as appropriate

​

    16. Is aware of, uses and promotes good safety practices, including but not limited to:

  • Body mechanics

  • Reporting of defective equipment or hazards

  • Wiping spills or wet floors immediately

  • Using Universal Precautions and appropriate infection control measures

  • Knowledge of emergency procedures

  • Reporting incidents/accidents to the Administrator on the day of occurrence

   17. Leads care conferences with other members of the health care team

   18. Leads the agency’s quality assurance program

   19. Demonstrates commitment to team building activities within company

   20. Maintains absolute confidentiality of all information pertaining to clients, families, and employees

   21. Promotes a safe and comfortable environment for the client and family

   22. Demonstrates excellent written and verbal communication skills

   23. Recognizes and manages own stresses, which may affect work performance

   24. Participates in in-service educational programs, as required by state regulations

   25. Performs additional duties as assigned by the Administrator

   26. Adheres to all agency policies

 

 

EXPERIENCE/EDUCATION/JOB SKILLS REQUIREMENTS

 

  • High School Graduate

  • Graduate of accredited nursing program and licensed as a registered nurse in the State of Minnesota

  • Experience in home care or assisted living preferred

  • Supervisory experience preferred

  • Must be able to read, write, comprehend and carry out directions and instructions

  • Able to function independently

  • Good health status and emotional stability

  • Demonstrate a strong commitment to service excellence
